The 2024-2025 Application is open as of May 1st 2024. If you had previously applied for a service provider user name and rate in this system, simply log in to the system to create a new application for the 2024-2025 school year. If you have never registered, please utilize the New Provider application to create a user name for your application. Once you have created a login ID, you may log back in to the system to create an application for a rate. This application closes at midnight on October 15th.
If you are an admin for multiple providers please just create one user login the first time you register a provider and use the login you created for all additional providers. There will be a button to use when filling out the provider information on the form. You will no longer need a user account for each provider and when you login, you will be able to select which provider you are wanting to manage.
